Learn where buyers discovered a brand and what helped them choose it. Launch a short survey on Shopify’s Thank you and Order status pages, with personal setup help from Dennis at OrderSurvey.
These are example research questions. Survey answers add buyer context; they do not establish that a channel caused a sale.
Try it before installing
A short question. A relevant follow-up.
Choose an answer in the demo and see how the next question changes. You can also try NPS and product-feedback examples.
The website demo uses the app’s branching engine. It is an example, not a live Shopify checkout; demo answers stay in this page and are not saved as merchant responses.

Personal help, from question to first response
Launch one client survey together.
Agree on a useful question
Tell us what your client wants to learn, what they already collect, and how your team will use the answers. We’ll check fit before changing their setup.
Set up and verify both pages
With the client’s approval, create the survey and save the OrderSurvey block separately on Thank you and Order status. Test each answer path and confirm a saved response.
Review the first answers
Separate test submissions from customer feedback. Review the question and response pattern together, then decide whether more volume or a Pro CSV export is useful.
A straightforward starting point
Free to start. $39/month when Pro fits.
Free includes 100 responses per rolling 30 days, first-order targeting and conditional questions. Pro adds unlimited responses, CSV export, webhook alerts and priority support.
A Shopify store, permission to install OrderSurvey and edit the checkout pages, and one business question to investigate. We can help your team choose the question, review the audience and test both survey placements with the client's approval.
Can we survey only first-time customers?
Yes. First-order targeting is included on Free and Pro. Add the Thank you block so Shopify can identify the order's audience. On Order status, targeted surveys appear only for orders already identified through that block; older or unidentified orders are excluded.
How does pricing work across clients?
Each store installs and manages its own OrderSurvey app. Free includes 100 responses across that store per rolling 30 days. Pro is $39 USD per store per month, billed through Shopify, with unlimited responses, CSV export, webhook alerts and priority support. There is no automatic paid upgrade.
Can our team work with the answers?
Yes. Review responses in each client's app, or use Pro's CSV export in your own analysis. Access depends on the permissions the merchant gives your team. There is no shared agency dashboard; confirm any required integration before choosing the app.
