Setup & support
Your first survey starts here.
Add a survey to your Thank you and Order status pages, understand your customers, and get help when you need it.
How do I get my survey live?
- Install OrderSurvey and open it in your Shopify admin.
- Create and enable a survey, or use the sample survey offered in the app.
- Open the checkout editor from OrderSurvey, or go to Settings → Checkout and customize your checkout configuration.
- Select the Thank you page, add the OrderSurvey app block, and save.
- Select the Order status page, add the block there too, and save.
- Check the preview on both pages and confirm your survey is enabled for the placements you want.
Installing the app does not automatically add a block to your published pages. Each page needs its own block. No theme code is required.
Can I show a survey only on a customer's first order?
Yes. In the survey editor, choose First orders only under Show survey to, then save. All orders and Repeat orders only are also available on Free and Pro.
Add the OrderSurvey block to your Thank you page. Shopify identifies whether the order is the customer's first completed purchase with your store, including purchases before you installed the app.
For later Order status visits, we use the classification captured on the Thank you page. Older orders and orders we have not identified will not show a first- or repeat-order survey. Add the block to both pages to cover this flow. Preview orders may not include this classification; confirm with a test order.
Why isn’t my survey appearing?
- Check that the survey is enabled and includes the page you are viewing.
- Confirm the OrderSurvey block was added and saved on that page in the checkout configuration your store uses.
- Review targeting rules. An order must match the rules for the survey to appear.
- If you are on Free, check your response usage. The limit is 100 responses across your store in a rolling 30-day period.
- A survey already completed for an order should not ask the customer to answer again on the other page. Use a fresh test order when checking the complete experience.
If it still does not appear, contact us with your store domain, the page affected, and the time you tested it.
Where do customers see the survey?
The Thank you page appears after checkout. The Order status page lets customers return to their order later. OrderSurvey provides a survey block for each page. It also supports Shopify POS as a separate experience.
What questions can I ask?
Use single choice, multiple choice, dropdown, short text, long text, star rating, or NPS questions. A common starting point is “How did you hear about us?” You can also ask about the checkout experience or why a customer chose your store.
A dropdown is one answer format inside the survey. The survey itself is the block on your Thank you or Order status page. For more ideas, see our survey question examples.
Can different customers see different questions?
Yes. Choose All orders, First orders only, or Repeat orders only. You can also target by order value, products, country, and currency. Conditional branching lets an answer determine which follow-up question appears next. Both are included in Free. Customer-tag targeting is not available on these checkout pages.
Can I show a survey only on a customer's first order?
Yes. Open your survey and select First orders only under Show survey to. This means the customer's first order with your store, including purchase history before OrderSurvey was installed.
The Thank you block receives Shopify's first-order classification. Order status reuses it for that same order, even after later purchases. If the Thank you block never received the classification, including for older orders, a first- or repeat-order survey stays hidden on Order status. Use All orders when you want to include those orders.
Where can I see the answers?
Open OrderSurvey in your Shopify admin to review responses and results. Pro includes CSV exports and configurable webhook alerts for low NPS answers. OrderSurvey does not currently include a native Klaviyo integration or send customer surveys by email or SMS.
What does Free include, and what happens at the limit?
Free includes unlimited surveys, both online placements, targeting, and branching, with up to 100 responses across your store in a rolling 30-day period. At the limit, new responses are not collected until usage falls below the limit or you upgrade. There is no automatic paid upgrade.
Pro is $49 USD/month and adds unlimited responses, CSV exports, webhook alerts, and priority support. See the full pricing details.
What happens to my data if I uninstall?
